First, a Notice to Resume Prior Name must be executed and filed with your local Prothonotary or Department of Court Records. Consult with a lawyer to ensure that you have executed the correct documentation to accomplish this.
Next obtain several copies of the certified copies of the Notice to Resume Prior name once filed with the Court.
Finally, update your important documents with the proper agency or authority. Start by updating your Social Security card with your new name, then you can easily update your driver’s license or state-issued identification card and your passport with your new name.
Once you’ve updated your Social Security card, driver’s license, and passport, you’ll need to notify other institutions and agencies of your name change. This may include banks, credit card companies, employers, insurance providers, utility companies, and any other organizations with which you have accounts or memberships.
